Appointments
We know you have many choices when choosing a Podiatrist in Piqua , OH , so we have made requesting an appointment a simple process via our Web site. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.

After Hours Emergency Calls
There is a physician on call at all times. When you need to reach the doctor after office hours or during the weekend, please call (800)838-8800 and ask for Dr. Polansky or Dr. Jones.

Fees, Billing and Insurance
Our fees for professional services are based upon the usual and customary charges for our area and reflect the cost of providing quality care. All charges are the responsibility of the patient or guardian from the date services are rendered. If you are covered by medical insurance we will automatically bill the insurance company as a convenience for you. It is your responsibility to know what is covered under your plan and to provide us with any changes in insurance or patient information at the time of registration. Your medical insurance policy may require you to pay a portion of the charges (your co-payment and/or deductible). Our receptionist can assist you in determining what you will pay. If you need to make other arrangements due to special circumstances, please discuss them with our billing staff prior to your appointment. For your convenience, we accept payment by check, cash, MasterCard and VISA.
